Building a custom review form
Custom review forms allow your organisation to align the Intuety review process with your own internal review form. When an approval reviewer carries out a review of a job or a risk assessment and method statement (RAMS), they will complete your custom form as part of the workflow.
If your company chooses not to build a custom review form, reviewers will use Intuety's default review form, which includes the mandatory reviewer comments, approve or reject decision, and expiry date fields.
Who can use thisYou must have the relevant company administration permissions to access the form builder and create or edit a custom review form. Approval reviewers will then use the published form when carrying out their reviews.
Accessing the form builder- Open the menu on the left-hand panel.
- Select Administration.
- Open the form builder.
Within the form builder you can:
- Give the form a name that aligns with your internal systems.
- Add a description to explain the purpose of the form.
- Create individual sections to group related fields.
- Add placeholder text, help text, and a default value to any field.
- Mark fields as mandatory or optional.
- Use Preview to see how the form will appear to reviewers before publishing.

- Checkboxes
- Decision tree
- Table
The following fields are required in every review form to align with the Intuety workflow and cannot be removed:
- Reviewer comments
- Approve or reject decision
- Expiry date
You can change the wording of these fields to match your internal terminology, but the fields themselves must remain on the form.
Tips and good practice- Group related questions into sections so reviewers can work through the form logically.
- Use help text to guide reviewers on what is expected in each field, especially for complex questions.
- Always Preview the form before publishing to check the layout and flow from the reviewer's perspective.
- Keep mandatory fields to the minimum needed — too many required fields can slow reviewers down.
- Only users with company administration permissions can access the form builder.
- The reviewer comments, approve or reject decision, and expiry date fields are mandatory and cannot be removed, although their wording can be changed.
- If no custom review form is built, reviewers will automatically use Intuety's default review form.
